Payment Options
Payment is expected at the time of service and can be made by cash, check, debit/credit card (American Express, Visa or Mastercard) and most Flexible Spending Account Cards.
Prescription Refill Policy
To refill our prescription, please call your pharmacy and ask them to have a refill request form faxed to LylesMD at 469-916-1824. Please anticipate the need for refill several days in advance so you will not risk running out of your medications. For some medications that require regular laboratory or other monitoring, it may be necessary to have a lab test or office visit prior to additional refills. Some medications cannot be refilled by phone and may require a written prescription to refill.
Medical Records Release
If you need to have medical records copied, please make your request as far in advance as possible. If you are requesting imaging or X-ray film, it will be necessary to obtain those from the imaging center where they were performed. Most specialists now want both the radiologist’s written report as well as a CD of the actual films or scan.
