A message from Dr. Lyles:

As of October 31, 2022, I will close the office of LylesMD. This decision is difficult to make. However, the time has come for me to tend to family matters and begin working on my own bucket list.

Retiring now is easier knowing that a qualified physician is taking over my practice and can assume your care. Dr. Cynthia Swayze-Smith will be moving into my current office with members of the same wonderful staff. Dr. Swayze-Smith offers an annual comprehensive wellness program through her affiliation with MDVIP, a national network of primary care doctors who focus on personalized and preventive care.  As it has been with me, you will be able to contact her 24/7.

Until October 24, you may request (in writing) a copy of your medical records for yourself or a copy to be sent to a physician of your choice. To cover the cost of copying records, there is a nominal charge of $25.

Your entire chart will be retained by Dr. Swayze-Smith in storage, and you will be provided with the most relevant items from your chart (e.g., reports, labs, x-rays, scans). Reports generated by specialists are not included in these copied records. Please contact specialists directly for their records.

Also, please request all prescription refills that are due to be refilled on or before October 24.

A most difficult aspect of closing the practice is the knowledge that I will not be seeing you in a professional capacity. Over the past 40 years, I have formed many strong and significant relationships with my patients. I am blessed to have had the opportunity to get to know my patients personally, along with many families.

Rather than tell you, “good-bye,” I prefer to say, “I will be seeing you around town.”
